*if your school has a media consent form on file, you are not required to fill out our form separately*

Head chaperones–please bring a list of any students who may not be photographed & event staff will give you stickers upon arrival for you to distribute to those students who may not be photographed to wear on the right side of their chest

Student Permission Slip

Optional Permission slip for students attending a Career Jam event. Downloadable in both PDF and Microsoft Word formats

Career Jam does not require a permission slip on our end. However, you may edit the Career Jam permission slip for your convenience. Your school is expected to send home any required paperwork for your district.
